Ohio Select Tryout Waiver & Release By registering for or participating in an Ohio Select tryout, showcase, workout, evaluation, or related activity, the participant and parent/guardian acknowledge and agree to the following: Assumption of Risk Participation in baseball and athletic activities involves inherent risks, including but not limited to running, throwing, hitting, fielding, sliding, diving, collisions, use of equipment, weather conditions, and other physical activity that may result in injury, illness, permanent disability, or death. Participation is voluntary, and all participants knowingly and voluntarily assume all risks associated with participation. Release of Liability In consideration for participation in Ohio Select tryouts and related activities, the participant and parent/guardian hereby release and hold harmless Ohio Select, its coaches, volunteers, organizers, trainers, affiliates, sponsors, facility operators, and representatives (“Released Parties”) from any and all claims, liabilities, damages, injuries, losses, or expenses arising out of participation in the tryout activities, including claims resulting from ordinary negligence to the fullest extent permitted by law. This release applies to all injuries, illnesses, damages, losses, or claims occurring during or related to participation in Ohio Select tryout activities. Medical Authorization The parent/guardian certifies that the participant is physically able to participate in athletic activities. In the event of injury or medical emergency, permission is granted for emergency medical treatment if the parent/guardian cannot be immediately reached. The parent/guardian understands they are solely responsible for any medical expenses incurred. Code of Conduct Participants and families are expected to conduct themselves respectfully toward coaches, players, officials, staff, and other families throughout the tryout process. Ohio Select reserves the right to remove any participant or spectator for unsafe, disruptive, inappropriate, or disrespectful behavior. Tryout Participation Participation in an Ohio Select tryout does not guarantee selection, placement, roster acceptance, playing time, future participation, or an offer to join an Ohio Select team, program, or organization. Ohio Select reserves the right to make all roster and organizational decisions at its sole discretion based on evaluations, team needs, player fit, availability, conduct, and organizational philosophy. Photo & Media Release I grant Ohio Select permission to use photographs, video, livestream footage, or other media captured during tryouts for social media, website, promotional, recruiting, or organizational purposes without compensation. Equipment & Personal Property Participants are responsible for their own equipment, belongings, and personal property. Ohio Select and the Released Parties are not responsible for lost, stolen, or damaged items. Acknowledgement By registering for or participating in Ohio Select tryouts, I acknowledge and agree that: I have read and understand this waiver and release I understand participation involves inherent risks I voluntarily agree to these terms I understand I am waiving certain legal rights I am the participant or the parent/legal guardian of the participant and have authority to agree to these terms on the participant’s behalf
